Use of Bandage in Muscular Alterations of Children Accompanied in a Cohort Design Zika – Jundiai

Abstract

We experience in Brazil a rapid process of dissemination of infection by the Zika virus, responsible for the increase in the number of cases of microcephaly among other complications, some of them muscular. The objective of this study was to describe the experience in the use of bandages in muscular alterations found in children accompanied the Project ZIKA-Cohort Jundiaí. Type of Study: Descriptive study, cut of  the ZIKV Cohort Study, started in March 2017 with completion in 2021. The infants were evaluated during the project task forces from March 2017 to March 2019. For evaluation of the neurodevelopment the Infant Neurological Assessment of Hammersmith was used, when identified the changes, the children were forwarded to initiate the protocol of bandage or received specific guidance. The mothers participating in the task forces were consulted regarding interest in participating in the project. Results: Sample composed of 86 infants, evaluated in 30 meetings of the task forces in the period from March 2017 to March 2019; of these, 25 infants (68%) presented muscular alterations, being the main, delay in the formation of the plantar arch - flat feet (9 - 36%), eversion of the foot (5 - 20%), valgus knee (3 - 12%), joint stiffness and tip of foot (2 - 8%), 14 (56%) were referred to physiotherapy, the other received guidance regarding the stimulation. The findings highlight the importance of early neurological assessment and the early detection of  muscular and articular changes  by physiotherapy. The infants who showed changes when treated early with the use of bandages evolved to a better prognosis.

Vivenciamos no Brasil um rápido processo de disseminação da infecção pelo Zika Vírus, responsável pelo aumento no número de casos de microcefalia entre outras complicações, algumas delas musculares. O objetivo do estudo foi descrever a experiência na utilização da bandagem nas alterações musculares encontradas em crianças acompanhadas no Projeto ZIKA-Coorte Jundiaí. Tipo de Estudo: Estudo descritivo, recorte do Estudo de Coorte ZIKV, iniciado em março de 2017 com término em 2021. Os lactentes foram avaliados durante os mutirões do projeto de março de 2017 à março 2019. Para avaliação do neurodesenvolvimento foi utilizada a Avaliação Neurológica Infantil de Hammersmith, quando identificadas as alterações, as crianças foram encaminhadas para iniciar o protocolo de bandagem ou receberam orientações específicas. As mães participantes do mutirão foram consultadas quanto ao interesse em participar do projeto. Resultados: Amostra composta de 86 lactentes, avaliados em trinta encontros do mutirão no período de março de 2017 à março de 2019, destes, 25 lactentes (68%) apresentaram alterações musculares, sendo as principais, retardo na formação do arco plantar – pés planos (9 – 36%), eversão do pé (5 - 20%), joelho valgo (3 - 12%), rigidez articular e ponta de pé (2 - 8%), 14 (56 %) foram encaminhados para acompanhamento fisioterapêutico, os demais receberam orientações quanto a estimulação. Os achados destacam a importância da avaliação neurológica precoce e da detecção precoce das alterações musculares e articulares pela fisioterapia. Os lactentes que apresentaram alterações quando tratados precocemente com o uso da bandagem evoluíram para um melhor prognóstico.
